Client Forms
The forms I need from you are not on this website. Instead, a link to those forms will be emailed to you when you schedule and I confirm your first appointment, and you will fill them out and submit them online.
What you need to know about my required forms
Timeframe to Submit
I will need all forms submitted at least 24 hours before our initial appointment. I will do my best to send you a reminder a few days before if I notice you haven’t completed them, but I can’t guarantee it. For that reason, I strongly suggest you complete all forms as soon as possible after you receive the email from me. If we get within that 24 hour window and I don’t have forms from you, I will cancel your session and you will be free to reschedule it for a date after the forms are submitted.
Who Needs to Submit
Anyone I’ll be seeing as a regular client. If you are seeing me for couple counseling, both partners will receive a link in your email, and I will need all requested forms from both partners.
Importance of Reading the Forms
I strongly suggest you read all forms carefully. The relationship between you and me will be different — much closer — than the relationship between you and a dentist, doctor, or other health care provider. It will be important for you to know exactly how I work and what you can expect, as we want to avoid an unexpected surprise causing a strain in our relationship. No relationship is perfect, and difficulties can of course be worked out in therapy, but best to avoid them whenever possible.
